12. Smokeless Tobacco (Snus) and Cardiovascular Disease : Associations with Heart Failure and Prognosis after Myocardial Infarction
Abstract : Previous investigations of snus use (oral moist snuff, a Swedish form of smokeless tobacco) and cardiovascular disease have generally focused on atherosclerotic events such as myocardial infarction and stroke, likely because smoking is such a well-established risk factor for atherosclerotic disease. Smokeless administration of tobacco circumvents most of the atherogenic effects of the combusted products from smoked tobacco, but it is possible that the potent autonomic and hemodynamic effects of snus and nicotine per se are detrimental for cardiovascular tissues. The ability of 24-hour VCG monitoring to differentiate patients with or without ischemic heart disease (IHD), to provide long-term prognostic information and to predict results from a predischarge exercise test in patients with unstable angina pectoris or acute myocardial infarction (MI) was evaluated. Abstract : The aim of this thesis was to study incidence and case-fatality rate (CFR) of myocardial infarction (MI) and ischemic heart disease (IHD), by exploring relations to socioeconomic position (SEP), other cardiovascular risk factors, medical seeking pattern and time trends. Data from the Malmö Myocardial Infarction Register (1986-1995; n=5533), the Malmö 1990 cohort (1990-2003; n=69 223), the Malmö Preventive Project (19742004; n=33 224) and out-of-hospital IHD deaths in southern Sweden (1992-2003; n=14 347) were used.
